Adding, changing, & removing forms in a process

To add a form to a stage, click on the stage in the status bar. Then click on the “Choose Form” button. A list of the forms in the system that are the same form type (LOI, Eval, App, Approval, Denial, FU) will appear.

  • For example: If you are in the LOI stage, when you click on “Choose Form,” all of the LOIs currently built in the system will be in the list.

You can choose to copy one of the existing forms into the process by selecting the radio button next to the form name and clicking “Copy Selected Form”.

  • If you do not want to utilize one of the forms in the list, click “Create New Form” to build a brand new form.
  • Once a form has been added, you can use the following tools:
    • The “Preview” button allows you to view a clean version of the form from the administrator’s view, similar to the form view that an applicant sees.
      • This view includes all questions on the form, including questions with admin only and internal visibility types which will not appear when an applicant is viewing the form.
      • If you would like to view the form as an applicant or a reviewer, select the role from the “Preview As” drop down.
      • Please note that you cannot edit a form when in preview mode.
    • The “Edit” button will take you into the “form builder” where you can edit the form. Form builder editing capabilities include:
      • Editing the form name,
      • Adding new questions and question groups,
      • Editing/deleting existing questions and question groups, &
      • Reordering questions and question groups
    • The “Remove” button allows you to remove the form from the process.
      • You cannot remove a form if the process is turned ON. It must be turned off while you remove the form, then the process can be turned back on.
      • Once a form is used in the process it is permanently attached to the process and it cannot be removed.
    • The “Change Form” button allows you to change the selected form to a different form.
      • When you click “Change Form” you are presented with the list of forms in the system that are the same form type. Remember that you can select a form from the list and copy it into the stage, or you can choose to create a new form:
      • Note that:
        • The Change Form option is not the button you use to edit a form.
        • You cannot change to a different form if the process is turned ON. It must be turned off while you change the form, then the process can be turned back on.
        • It is important to note that once a form has been used it is permanently attached to the process and it cannot be changed to a different form.
